All of the permission slips for Troop 809’s trips during the remainder of 2014 are available under the forms tab as well as through the links below! 2015 permission slips will be available in the near future – we are still waiting on confirmations for some of the dates/locations before those slips are distributed.
December – Please note a separate waiver form is required by Sky Zone for all participants of this event and MUST BE DONE ONLINE PRIOR TO ATTENDING THE EVENT! Please CLICK HERE to go to their webpage to complete the waiver. I suggest you complete the waiver when you print the permission slip for the event!
Again, we please ask you to print these permission slips (you can use Adobe Reader to complete the form and then print it!) and return them with your payment by the appropriate registration deadlines. We will only have a couple forms at the Troop meeting. If you do not have the ability to print the forms yourself, please let the Scoutmaster or webmaster know and we will get forms to you!
